About the role

cohealth is seeking a passionate Occupational Therapist to join our team. You will be responsible for evidence based, client centred assessment and care for people applying for NDIS eligibility and people already successfully on the NDIS having a wide range of medical conditions and disabilities. You will be required to complete a holistic assessment to facilitate applications for NDIS eligibility, as well as to determine appropriate therapeutic options for a varied case load, assisting children through to adults within the community.

 

Your duties include:

  • Providing evidence-based, client centred and flexible Occupational Therapy service to clients including people from marginalised, vulnerable and CALD backgrounds
  • Providing timely services across a range of funding sources including the NDIS and HACC-PYP in line with agreed daily targets
  • Developing, reviewing, and evaluating Occupational Therapy direct care and health promotion programs using a codesign approach
  • Maintaining client files and statistical/program records in keeping with the organisation’s policy and funding requirements
  • Having appropriate knowledge of the funding agreements underpinning their role

Our ideal candidate:

  • Tertiary qualification in Occupational Therapy
  • Proof of qualification to be sighted
  • Registered with Australian Health Practitioner Regulation Agency (AHPRA) or intend to register
  • Demonstrate clinical reflection skill utilisation and translation of knowledge to inform clinical care.

All Healthcare workers in Victoria are required to have received three doses of COVID-19 vaccination. Proof will need to be sighted during the recruitment process.
