About the Opportunity

Part of Australia’s largest not-for-profit Catholic health and aged care network, St Vincent’s Health Australia, SVHM provides a diverse range of adult clinical services including acute medical and surgical services, sub-acute care, medical diagnostics, rehabilitation, allied health, mental health, palliative care, correctional health and community residential care.

This position is part of the Health Care for the Homeless (HfH) Program. HfH comprises several services that support clients that are experiencing or at risk of homelessness. HfH aims to integrate and better coordinate existing services within St Vincent’s Hospital and where service gaps are identified, support the development of new services to meet the needs of our clients.

Operationally the role is based at Frontyard Youth Services which is a specialist youth service located in Melbourne’s CBD. Frontyard aims to support young people aged 12 to 24 to meet their physical, emotional and social needs and to develop pathways out of homelessness. This includes but is not limited to providing 18 beds crisis accommodation service and drop in centre housing multiple services working with young people across Greater Melbourne and throughout Victoria.

This role has dual responsibilities including to:

  • provide clinical leadership for the SVHM Check‐In staff based at Frontyard Youth Service ensuring delivery of an efficient, effective and clinically therapeutic service. The Senior care coordinator will manage day to day clinical operations of the Check‐In staff, rostering, monitor and support team activity achievement, support recruitment activities, staff orientation and staff performance development, provide clinical and professional advice and supervision to the team, lead case conference and other team specific meetings, and ensure the staff operates within accepted best practice guidelines.
  • This role will also manage a small caseload of clients within the Check‐In program and provide brief opportunistic support to users of the drop in service.

This position will receive clinical/professional support from through the ALERT leadership team. Operational leadership onsite will sit with Frontyard.

About you

We are looking for someone who can:

  • Together with Frontyard, SVHM and Team leadership, ensure delivery of an effective and responsive service for clients of the Check‐In program and support other team members, as clinically appropriate in accordance with SVHM guidelines.
  • In conjunction with the care coordinators actively screen and engage with clients within the accommodation service and drop in centre, using holistic comprehensive assessments based on the clients’ health and wellbeing needs.
  • Perform the duties of the position to the best of their ability and to a standard acceptable to SVHM.
  • Ensure all health and safety related policies, procedures and directions are complied with in the area they manage.
  • Actively manage individual and team performance to maximise performance and minimise workplace problems and conflict.

Essential:

  • Nursing qualification with minimum 7 years experience in relevant acute/community setting
  • Eligible for registration/membership with AHPRA
  • Previous experience in a similar or related role, including working within a care coordination framework with people with complex psychosocial needs
  • Knowledge and experience in one or more of the areas of homelessness, mental health, substance use, young people with complex needs and/or disability 
  • Demonstrated understanding of issues impacting youth health and wellbeing
  • Highly developed interpersonal skills incorporating excellent verbal and written organisational and negation skills

Screening Criteria

  • National Police check
  • Working with Children check and NDIS Worker Screening check or ability to obtain
  • Valid Victorian Drivers licence
  • Right to Work in Australia
  • Up to date with COVID-19 Vaccinations or valid medical exemption
